Job summary

The post is within a multidisciplinary team, which assesses, evaluates, and initiates management for older people admitted acutely to the Hospital at Home service to improve their outcomes and experience of care.

Main responsibilities

  • The posts are Specialty Doctor whole time appointments for 10 sessions with the Western Health and Social Care Trust. (2 posts based in Gransha Park Site and 1 post based in Erne Health Centre). The work pattern will be Monday to Friday between the hours of 9am to 8pm on a shift pattern to be agreed and the Consultant will supervise clinical practice. There is an expectation of providing extended evening medical cover when a planned enhancement of the H@H service commences. Medical cover will be provided up to 10:00 p.m. on weekdays and from 1:00 – 5:00 p.m. on weekends and bank holidays.
  • Core duties will consist of the assessment and management of acute elderly patients admitted to the Hospital at Home service.
  • The Specialty Doctors for the H@H service will assist the Consultant in receiving direct referrals from GPs and other primary care providers including NIAS, Community Respiratory Team and Nursing Homes and accepting appropriate referrals onto the H@H service to prevent admission to hospital.
  • The Specialty Doctors, in collaboration with other H@H team members, will provide Comprehensive Geriatric Assessment of people over 65 admitted to the H@H service in the patient’s own home, residential home or nursing home.
  • The aim of the Specialty Doctors and H@H team will be to provide quality care and treatment, prevention of hospital admission through a case management approach to promote safe and effective discharge.
  • The Specialty Doctors will be expected to undertake administrative duties associated with the H@H service.
  • Any changes in the working arrangements will be by means of discussion and agreement (which should not be unreasonably withheld) with the Specialty Doctors
