Contract and Hours: To establish a waiting list for permanent, temporary, full-time and part-time vacancies that may arise over the next 12 months

Job summary

The Pharmacist appointed will be a member of the team providing a pharmaceutical service to the Western Trust. He/ She will work as a specialist clinical pharmacist providing a high level of clinical pharmacy services to the Integrated Medicines Management Service.

 

Main responsibilities

  • Support the provision of pharmacy services in line with Medicines Management Controls Assurance Standards, the Medicines Optimisation Quality Framework and RPS Professional Standards for Hospital Pharmacy Services
  • Provide a high level of clinical pharmacy input, as part of the multi-disciplinary team to all patients within designated wards, to ensure maximum efficacy, safety and economic drug use to demonstrate professional accountability and to enhance patient care
  • To ensure the appropriate management of patient’s own drugs
  • To deliver a high quality, specialist ward based pharmacy service, including expert review of complex drug regimens, medicines reconciliation, drug interactions monitoring, adverse drug reaction monitoring and ensure timely and efficient drug supply
  • To carry out effective medication discharge planning and provide appropriate patient information to patients and primary healthcare professionals
  • To become expert and demonstrate a high level of knowledge of medicines usage within a specialist clinical area
  • Ensure systems are in place to ensure that medicines are used appropriately, safely, and cost-effectively in accordance with Trust policy, standard operating procedures and medicine legislation and carry out routine audit to provide assurance
  • To develop and ensure adherence to evidence based prescribing practice guidelines
  • To make evidence based decisions, in partnership with patients and other professionals within the specialist area
  • To make clinical decisions where limited information is available
  • To provide support on concordance related issues to patients and carers
  • Demonstrate the ability to provide safe, clinically effective and cost-effective use of medicines in accordance with Trust policy, standard operating procedures and medicine legislation. This will include prescription review, advising on dosage and side effects and on the safe administration of medicines
  • Ensure the security of medicinal products in all settings including wards and clinics and the main pharmacy department
  • Promote research activities of team, by identifying areas for audit and service redesign. The post holder will undertake and publish practice research and enable team members to do likewise. The post holder is expected to determine how the outcomes are best achieved in specific circumstances
  • To practice as an independent pharmacist prescriber, working within parameters of prescribing agreed by the Trust’s Non- Medical Prescribing Working Group and a supporting governance framework
  • Participate in weekend, bank holiday and extended hours working and participate in the pharmacy on call working arrangements
