Role summary
The Condition Management Programme (CMP) is a voluntary, short-term work focused programme delivered by health care professionals throughout the five health care trusts in NI. It is funded by the Department for Communities, and assists service users to develop strategies and supports to manage their health condition(s) and make progress towards a return to work. CMP is also available for people in work but struggling to stay in work and needing support with health conditions.
The post holder will provide professional Occupational therapy input to the Condition Management Programme utilising expert skills in assessment of health, social and occupational performance with clients referred to the programme. The post holder will be expected to work independently and manage a caseload of clients with complex needs across a range of health conditions using evidence-based/client centred principles to assess, plan, implement and evaluate interventions.
The post holder will work inter-professionally in a multi-disciplinary team. They will carry out assessment, planning and implementation of CMP to clients referred. They will be required to work in partnership with CMP colleagues, Department for Communities Work Coaches, other health and social care professionals and voluntary and statutory agencies, in the best interest of the clients and help to support clients for whom a return to/maintaining work is a viable option.
Main responsibilities
- To provide an Occupational Therapy Service to clients in the Condition Management Programme (CMP) using advanced clinical reasoning skills, in accordance with local and national guidelines and standards of practice.
- The post-holder will be responsible for the assessment, intervention, planning and delivery of Occupational Therapy input to CMP clients.
- The post holder will work as an autonomous practitioner and provide an advanced heath, social and occupational assessment of clients presenting with various health conditions, including but not limited to mental health, musculoskeletal and cardiorespiratory conditions.
- To use clinical skills to identify occupational performance deficits and the barriers to improved health status and potential employment.
- To undertake a client centred approach by working in partnership with the client in problem identification and formulation, goal setting and action planning.
- To facilitate an improvement in the client self-management of their health condition and in turn to improve their functional capacity.
- To assess clients with combined physical and mental health needs and recommend basic aids/ equipment to promote independence as appropriate.
- To provide condition management advice through 1:1 case management and/or group interventions and/or onward referral to appropriate services.
- To provide an advisory service to clients, the client’s wider social network, other health professionals, the primary healthcare team including GPs and representatives of Jobs and Benefits Office and liaison with voluntary and statutory agencies where appropriate.
- To work in partnership with, Jobs and Benefits Office staff – Work Coaches, other health professionals and voluntary agencies as appropriate to deliver CMP.
- To be responsible for the organisation and day-to-day management of own caseload.
