Job summary

To support the existing Consultants in Care of the Elderly and Palliative Care to provide medical cover to the inpatients in Rehab and Palliative care wards OHPCC. This will be one full time post to recognise the changes in complexity and turnover of patients in OHPCC.

Key responsibilities

  • The post is a whole time appointment for 10 sessions with the Western Health and Social Care Trust, based at OHPCC Palliative Care and Rehab wards, which have 40 beds in total. The initial work pattern will be on the basis of a *9.00 a.m. – 5.00 p.m. working day Monday to Friday and clinical practice will be supervised by the Clinical Lead, in their absence, by the Consultant in Rehabilitation Medicine alongside other Consultant colleagues.
  • Core duties will consist of the initial admission, assessment and management of patients admitted to the OHPCC beds, at the time of admission and during their hospital stay.
  • The post holder will assist Consultants and other colleagues in accepting appropriate referrals into OHPCC Hospital.
  • The post holder, in collaboration with other MDT members, will provide Comprehensive Geriatric Assessment and management of people over 65 who are admitted to continue their inpatient treatment pathway.
  • The aim of the successful candidate and team will be to provide quality care and treatment, through a case management approach to promote safe and effective discharge.
  • The successful candidate will work in partnership withal colleagues to reduce the length of stay in hospital and enable the patient to return home. Improved functional outcomes where possible is anticipated for pro-active discharge planning with involvement of primary and community colleagues.
  • The post holder will work in collaboration with the clinical pharmacist to deliver Medicines Optimisation. The overarching aim will be to proactively reduce treatment burden and inappropriate polypharmacy in older people, by deploying and managing patient-centred review and de-prescribing strategies during the patient’s admission
  • The post holder will work in close collaboration with Care of the Elderly colleagues to ensure appropriate admission to acute Care of the Elderly wards where necessary.
  • The post holder will participate in a weekly outpatient clinic within the department
  • The post holder will assist in both under and post-graduate teaching and will have the opportunity to develop or join in research projects, e.g. related to Frailty, Falls, Fracture Risk, Osteoporosis etc. Dedicated time will be allocated for this. The Trust has embarked on a process of recruitment for Teaching Fellowships for Specialty Doctor Grade clinicians to enhance the medical education programme at under and postgraduate level. The proposed job plan offers development opportunities for those who wish to pursue further studies in the care of Older People.
  • The successful candidate will be liable to deputise, as far as is practicable, for absent colleagues of the same grade.
  • The successful candidate will be expected to participate in multiprofessional audit and quality improvement, as well as educational activities within the Department.
  • The successful candidate will be expected to undertake administrative duties associated with the post.
  • Any changes in the working arrangements will be by means of discussion and agreement (which should not be unreasonably withheld) with the post holder.
  • The successful candidate will be encouraged to attend post-graduate education sessions within the Trust. 1.0 PAs per week will be devoted to continuing medical education / supporting professional activity including participation in local audit and quality improvement work. Study leave will be available at the discretion of the Clinical Medical Consultant and career guidance and advice on further medical education will also be available via discussion with the Clinical Director and other Consultants within the Directorate. This is a clinical post with no commitment to teaching or research. Additional SPA time may be allocated for other activities, such as educational supervision.
