Contract and Hours: Full-time (37.5 hpw) for 6 months initially 

 

Job summary

The post holder will be responsible for the provision of a high quality, effective and efficient Occupational Therapy service to clients referred within the Unscheduled Care Team which includes Emergency Department and MASU ward 1.

The post holder will be required to support Occupational Therapy Service delivery within these areas as directed by the Occupational Therapy lead. This may involve assisting in other areas of the hospital if the need arises. The post holder will manage autonomously, a caseload of patients with complex clinical needs at all times organising and prioritising workload in accordance with agreed policies and procedures.

The post will require integrated working with OT staff in other acute wards in prioritising patients and ensuring delivery of the service over 7 days. The post holder will also be responsible for organising and participating in the 7-day service as well as potentially in the future extended hours cover with in Unscheduled Care service.

The role of the Occupational Therapist within Unscheduled Care incorporates a range of assessment and interventions to support the clinical decision making regarding admission, managing client needs to enable discharge and to provide community follow-up and liaison to prevent readmission.  The post-holder will work inter-professionally and liaise closely with colleagues from the Acute, Community and Intermediate Care Sectors, with the responsibility for onward referral to appropriate services.

 

Main responsibilities

  • To initiate, administer and interpret clinical observations, comprehensive standardised and non-standardised assessment of clients with a range of health and social care needs and multiple pathologies, using advanced clinical reasoning, analytical and communication skills to determine likely prognosis of condition.
  • To formulate and implement Occupational Therapy treatment plans based on current evidence and a sound knowledge of available treatment options.
  • To independently manage a caseload of patients with complex and highly complex needs evidencing a high-level problem solving and reasoning skills approach.
  • To work as part of the multidisciplinary team to deliver Occupational Therapy interventions to clients referred.  
  • To utilise high level interpersonal skills to encourage, motivate and empower clients to actively participate in their treatment plan and rehabilitation programme.
  • To use highly developed communication techniques with clients who may have altered perception, dysphasia, hearing impairment or mental health presentations.
  • To supply / manufacture hand orthosis`s to individual specifications and to train the client/carer in the appropriate use of same.
  • To carry out home visits when required and to assess and arrange for the provision of specialist equipment and/or minor adaptations within the home setting.
  • To liaise with AHP’s, other Health & Social Care Professionals and related statutory, private and voluntary organisations, providing written and verbal reports and onward referrals where necessary.
  • To prioritise, manage and review caseload in accordance with service priorities.
  • To attend team meetings and inform the interdisciplinary team regarding the client’s rehabilitation progress, goal attainment and alterations to programme
  • To complete and participate in in-service teaching to other AHP staff and nursing staff
