Job summary

The CMP Physiotherapist will bring professional expertise to the team in the management of musculoskeletal, cardio-respiratory and neurological conditions (amongst others) for clients accepted onto the programme. The post holder will take a lead role in identifying and implementing the physiotherapeutic aspects of care to improve function and with the overall aim of facilitating a progression towards/ return to / sustain work.

The post holder will be expected to work independently and manage a caseload of clients with complex needs across a range of health conditions, using evidence-based / client centred principles to assess, plan, implement and evaluate interventions.

The post holder will work inter-professionally in a multi-disciplinary team. They will carry out face-to-face assessment, planning and implementation of CMP to clients referred. They will be required to work in partnership with CMP colleagues, Department for Communities, Work Coaches, other health and social care professionals, voluntary and statutory agencies in the best interests of the clients.

In conjunction with the CMP Co-ordinator, other Team members and Professional Lead the post holder will review and develop the service as deemed necessary to meet objectives set by the Department for Communities.

The post holder will be expected to work collaboratively with the CMP Coordinator to ensure effective utilisation of resources

Main responsibilities

  • To be responsible for the management of own caseload by ascertaining suitability of allocated referrals through assessment of clients including but not limited to musculoskeletal, cardio-respiratory, neurological conditions. This will necessitate: - identifying health, physical, psychological, social and occupational barriers to the client’s ability to return to work. - providing appropriate specialist input to develop in agreement with the person, functional goals and an action plan in order to assist the self-management of the person’s health conditions.
  • To work as an independent practitioner and provide an advanced biopsychosocial and physiotherapy assessment using evidence based practice for clients presenting with multiple health conditions including, but not limited to, musculoskeletal, cardio-respiratory and neurological conditions.
  • To establish, develop and maintain purposeful therapeutic relationship between client and practitioner.
  • To empower individual clients to understand and manage their condition using the principles of cognitive functional therapy, motivational interviewing skills and other behavioural modification therapies.
  • To monitor, evaluate and modify interventions and use outcome measures in accordance with clinical governance to measure effectiveness of the CMP.
  • To maintain high quality clinical records and provide reports for the service as required.
  • To provide effective supervision, guidance and support to junior staff e.g. CMP Exercise Practitioner.
  • To ensure that ongoing personal development and professional education needs are identified and met.
  • To liaise with professional and voluntary agencies in a specialist advisory capacity.
  • To source and provide supporting materials and literature to facilitate continued client self-management.
